Initial Situation
56-year-old patient presented with advanced gum disease, loose teeth, and a recent extraction of a compromised tooth.
The patient’s dental history revealed a progressive decline in oral health, with mobile teeth and ongoing challenges managing periodontal disease. The patient’s overall health was noted to include several general health concerns, which further complicated the management of their dental issues.

The Treatment Plan
Phase 1: Pre-Surgical Preparation
Prior to surgical intervention, the patient underwent a full-mouth hygiene appointment to reduce bacterial load and optimise oral conditions ahead of implant placement. No deep periodontal therapy or extractions were performed during this phase, as the patient was planned for full clearance under general anaesthetic.
This preparatory clean was essential in supporting post-operative healing and reducing the risk of infection following full arch surgery.
Phase 2: Full-Arch Implant Rehabilitation – All-on-4 and All-on-6
Following comprehensive treatment planning and consultation, the patient proceeded with full-arch rehabilitation under general anaesthetic. A complete dental clearance was performed at the time of surgery, followed by the placement of an All-on-4 prosthesis in the lower arch and an All-on-6 in the upper arch.
Despite initial concerns regarding bone quality—particularly in the upper arch—implant placement was achieved successfully. Careful planning, implant positioning, and adherence to surgical protocols allowed for excellent primary stability and successful integration.
The Outcome
The patient is extremely pleased with the result, reporting restored confidence, improved ability to eat, and no post-op complications. Further evaluation of the periodontium and supporting bone will continue through routine reviews.
